To book an appointment with Dr Carey you will first need to get a referral from your local doctor.

Once you have a referral, please complete the online registration form here. This will take between 10 and 20 minutes to complete, and you will be able to upload a copy of your referral as part of the process.

If you are seeking medical assistance with diabetes or weight management issues, you will also need to complete a three-day food diary prior to your first appointment. Food diaries can be recorded on a smartphone or tablet via the Easy Diet Diary app (recommended), or manually on our approved template (Microsoft Word format). Please note that in order to complete a medical-grade analysis, specific and accurate details are required – we therefore cannot accept food diaries in any other app or template.

After your registration is finalised, our team will make contact with you via phone to make your booking.

If you are attending for weight loss or diabetes management, we will endeavour to book your first three appointments in a space of 3-4 weeks. These initial appointments include metabolic and endocrine testing, development of a treatment plan, and an initial assessment/advice consult with the dietitian. (Our weight loss program then requires patients to commit to a program of review appointments every 6 weeks for 9 to 12 months.)

If you are seeking appointments for other medical conditions, your initial appointments will be booked according to your specific needs.

FAQ

Why do we need a referral before an appointment can be made?

Your referral includes important information in regard to your medical condition/s. This information allows the appropriate appointment times to be made. In addition, a current referral is necessary in order to claim a Medicare rebate. 

Please be aware that you are responsible to check when your referral expires during treatment, as any appointments without a current referral on file will be ineligible for a Medicare rebate, and any testing done onsite will be privately billed rather than bulk billed.Referrals from General Practitioners (GPs) are valid for 12 months. Referrals from Specialists are valid for 3 months.

What if my doctor has sent my referral directly to you?

If your GP has sent us your referral directly please email us to confirm we have received your referral. We will then be in contact with you shortly. 

 

How can I speed up the process?

Before your initial appointment you will need to fill out a Medical History Questionnaire - Click Here. In order to speed up the process we ask you fill it out while you wait for an appointment time. 

If you require assistance with weight or diabetes then we ask that you also complete a 3-day Food Diary. Our dietician will analyse your diet, and Dr Carey will use this information to plan and personalise your care.
